Patient Intake Form Policy

Equilibrium Chiropractic requires all new patients to complete a Patient Intake Form before their first appointment.

• If the form is completed online in advance, you’ll be able to spend more time with the Doctor during your visit.
• If the form is not completed ahead of time, we kindly ask that you arrive 15 minutes early to your appointment so our staff can assist you with completing it.

This process helps us provide you with the most efficient and personalized care possible.

What to Expect at Your First Appointment

At Equilibrium Chiropractic, Dr. Stafford takes a very thorough approach. His goal is to identify the root cause of your symptoms, rather than simply providing a temporary “band-aid” solution.

• Patient History: A consultation and pertinent medical history, review of systems, allergies, medications, current illnesses, and surgical history will be completed prior to any prescribed treatment.
• Examination: A detailed exam will be performed to evaluate your condition. This will included visual inspection, gait analysis, functional movement tests, 3D-Foot Mapping, Range of Motion, Chiropractic, Orthopedic, and Neurological Testing.
• X-Rays: X-rays are typically taken unless you already have recent images (within the last two years) and Dr. Stafford determines they provide the necessary information. Please bring any relevant X-rays, MRIs, or medical records with you. We do not take imaging of individuals that are pregnant. We do not usually recommend x-rays for young children unless to identify spinal pathology like scoliosis.
• Personalized Treatment Plan: At Equilibrium Chiropractic, every patient is unique and there is no “one-size fits all” approach to treating our patients. Our treatment plans are customized to the patients’ specific needs to help them achieve optimum health and vitality.

You will then return for your Report of Findings visit. During this appointment, Dr. Stafford will review your results and prescribed Treatment Plan. If you agree with the plan, we’ll begin your wellness journey together, helping you get back to feeling your best and living life to your fullest potential.
